See the following - a really fascinating study from Japan, where these very small carcinomas are often seen. Their protocol is to offer patients with papillary microcarcinoma close followup as a treatment option; unless the tumor has some high risk factors - like high grade pathology, lymph node metastasis, mulitocality, etc. In this patient population, only 6.7% of tumors enlarged by 3mm or more over a 5 year period. The 'take away point' is that papillary microcarcinoma of the thyroid does not require aggressive treatment.
